The undersigned petitioner alleges: ~ ~`~ -V ='

1. Petiitoner's residence is INDIAN RIVER COUNTY, Flor`ida,
and petitioner's post office address is 760 15th Avenue, Vero
Beach.
2. VIRGINIA AVEDUTI is an incompetent, whose date of
birth was August 14, 1899, who is 81 years of age, and whose so-
cial security number is 061-18-0263. The residence of the incom-
petent is St. Lucie County, Florida, and the post off ice addres
is 1709 B. South 17th Circle, Fort Pierce.
3. The nature of the incompetent's incapacity is organic
brain syndrome; chronic, non-psychotic, as adjudged by the Circuit
Court in Fort Pierce, St. Lucie County, Florida on February 13,
1981.
4. It is necessary that a guardian of the person ~--~re-
~~ be appointed for the incompetent.
5. The approximate value and description of the incompetent's
property are: social security monthly income of approximately
$415.16.
