S. S. #263-35-7435 ' Defendant. ~ . THIS CAUSE having come on for trial upon the pleadings filed herein and all parties having received proper and timely notice; the Court having heard testimony and/or considered the pleadings, papers, affidavits and other papers filed herein, and heing otherwise fully and well advised in the premises, it is ORDERED AND ADJUDGED as follows: 1. That the minor child(ren) DINET BERI.IN PASCO, d.o.b. 10/10/74, _ is/are declared to be the legitimate child(ren) of the Defendant MARLIN PASCO, and ANNETTE CLARK, the natural mother. 2. That the natural mother, ANPIETTE CLARK, shall have custody of the said child(ren) subject to the Defendant's right of reasonable visitation. ' 3. That commencing on MAY 7, 1983, the Defendant/ Father shall pay child support for and on behalf of said child(ren) in the amount of $10.00 per week, plus $2.00 statutory fee. All payments shall be made in cash, money order or cashiers check. All money orders and cashiers checks shall bear the payees name and Social Security Number and shall be made payable to the CLERK OF THE CIRCUIT COURT, and sent to: CLERK OF THE CIRCOIT COURT, S[SPPORT DEPARTAtENT, POST OFFICE BOX 700, FORT PIERCE, FLORIDA 33454. Said amount shall be' remitted monthly by the Clerk to the Department of Health and Rehabilitative Services, Child Support Enforcement Unit, 1317 Winewood Boulevard, Tallahassee, Florida, 32304. 4. That the Clerk of the Circuit Court shall and is hereby ordered to continue to transmit support payments received from the Defendant until ; further order o£ this Court or receipt of Notice to Discontinue Payments from ~ the Department of Health and Rehabilitative Services, in which event the ~ support payments shall thereafter be directed and payable to the aforesaid i natural mother or person having custody of the child(ren). 'i 5. That the above-named Defendant having been adjudicated the ~ father of the above-named child(ren), the DEPARTMENT OF HEALTH AND REHABIT.ITATIVE SERVICES, BURF.AU OF VITAL STATISTICS, AMENDMENT UI3IT, shall and it is hereby ordered to ar~end the above-named chil.d's/children's birth certificate(s) to show the above-named father's name. 6. Additionally, it is hereby ordered pursuant to Chapter 82-140, Laws of Florida (1982), that the employer of the Respondent sha12 deduct the sums herein provided on a periodic basis from the income due the Respondent and remit same, after deducting its charge for handling, to the Court " Registry. Sucli income ded~~ction shall not be effective until the Respondent is delinauent in two (2) child support payments and has failed to pay all arrearages and past public assistance obligations. Service of this order on the employer by the office of Child Support Enforcement of the Florida ; Department of Health and Rehabilitative Services shall be prima facie ~ evidence that the above conditions have occurred. Upon rec2ipt of this ' ~ order, the employer shall govern himself accordingly and remit payment forthwith. This income deduction order shall be in addition to, not in lieu of, all other remedies provided herein or hereafter. 7.
