THIS CAUSE }iaving come on for trial upon the pleadin~s filed herein
and all parties having received proper and timely notice; the Court having
}~eard testimony and/or considered the pleadings, papers, affidavits and other
papers filed tierein, and being otherwise fully and well advised in the
premises, iG is
ORDERED AND ADJUDGED as follows:
1. That the minor child(ren) ANTENETTE hICIE I.EE, d.o.b. 1/20/81,
is/are declared to be the le~itimate child(ren) of the I)efendant ALBF.RT ~1.
AhDERSON, and SHEII.A LEE, the natural ~r~other.
2. That the natural mother, SHF.ILA LEF., shall have custody of [he said
child(ren) subject to the Defendant's right of reasonable visitation.
3. That commencing on MARCH 4, 1983, the Defendant/ Father stiall pay
child support for and on behalf of said child(ren) in the amount of $1~.00 per
week, plus $2.00 statutory fee. All payments shall be made in cash, c~oney order
or cashiers check. All money orders and cashiers checks shall bear the payees
name and Social Security humber and shall be made payable to the CLERK OF THE
CIRCC?IT COCRT, and 5ent to: CI.ERK OF THE CIRCUIT COURT, SUPPORT DEPARTMENT, YOST
OFFICE BOX 700, FORT PIERCE, FLORIDA 33454. Said amount shall be remitted
' monthl~ by the Clerk to the Department of Health and Rehabilitative Services,
; Child Support Enforcement Unit, 1317 Winewood Boulevard, Taliahassee, Florida,
t 32304.
~ 4. That the Clerk of the Circuit Court shall and is hereby ordered
: to continue to transnit support payments received from the Defendant until
? further order of this Court or receipt of Notice to Discontinue Payments from
~ the ilepartment of F~ealth and Rehabilitative Services, in which event the
~ support payments shall thereafter be directed and payable to the aforesaid
~ natural motl~er or person ha~ ing c~istody of tt~e child(ren) .
5. That the above-named Defendant having been adjubicated the
~ father of the above-named child(ren), the DF.PART*IENT OF HF.ALTH AND
~ REt~ABILITATIVE SERVICF.S, BUREAL' OF ~'ITAL STATISTICS, AMF.tiDME:dT UNIT, shall
~ and it is hereby ordered to anend the above-named child's/children's birth
certificate(s) to show the abo~~e-named father's name.
~ 6. Additionally, it is hereby ordered pursuant to Chapter 82-140,
4 Laws of Florida (19£S2), that the employer c~f the Respondent shall deduct the
sums herein provided on a periodic basis frc,m the income due the Respondent
~ and remit same, after deducting its charge for handling, to the Court
~ Registry. Such income deduction shall not be effective until ttie Respondent
~
is delinquent in two (2) child support payments and has failed to pay all
arr2arages and past pubiic assistance obligations. Service of this order on
the employer b}~ the office of Child Support EnforcAr~ent of the Florida
Department of Health and Rehabilitative Services shall be prima facie
e^idence that the above conditions have occurred. ~pon receipt of this
order, the employer si~all govern himself acc~rdingly and remit payr~ent
forthwith. This income deduction order shall be in addition to, not i:~ lieu
- of, all other remedies provided herein or hereafter.
