That commencing on MARCH 15, 1983, the Respondent shall pay $25.(10 per month, plus $2.00 sta[utory fee, until tt~e AF~C is reimbursed in the amount of Si88.50. Said amount includes all arrears due herein. All payments shall be . made in cash, money order or cashiers check. All meney orders and cashiers checks shall bear the payees name and Social Security Number and shall be made paYable to the CLF.RR OF THF. CIRCUIT COURT, and sent to: CLERK OF CIRCUIT COURT, SUPPORT DEPA~tThtEtiT, P. 0. BOX 700, FORT PIERCE, FI.QRIDA, 33454. Said amount shall be remitted monthly by the Clerk to ttie Department of Health and Rehabilitative Services, Child Support Enforce~~ent Unit, 13I? t~'inewood Boulevard, Tallahassee, Florida, 32304. 2. That the above reimbursement payment shall be in addition to the regular support payment hereinbefore ordered by this Court and said payment shall continue until the minor child(ren) reaches majority, dies or marries. Said ~ regular support payr~ent shall be ser.t bv the Clerk of Circuit Court to the t custodian of the minor child(ren) or in accordance with the most re~ent noti.ce i filed bv the Department. However, should the AFDC monevs be reinstated, the r Petitioner shall file a notice of such reinstaterient and the Clerk shall forward f ~ regular support payments to the Depar[ment or in accordance with the most recent ~ notice filed by the Department. For the tir~e period the custodian ~f the minor k child(ren) is receiving AF~C moneys, the reimbursement payment as set forth above g shall be deferred. Ljpon cancellation of AFDC mane~•s paid to, or on behalf of, ~ the child(ren) the Department sha ll file a notice and the Respondent shall immediately commence paying [he reimbursement payment as set forth above. Ttie ~ Clerk shall forward said payments in accordance with the most recent notice iiled ~ bv the Department. ! ~ 3. Additionally, it is hereby ordered pursuant to Chapter R?-140, Laws ~ of Florida (1982), that the employer of the Respondent shall deduct the sums ~ herein provided on a periodic basis from the income due tl~e Respondent and remit same, after deducting its charge for handling, to the Court Registrv. Suct~ income deduction shall not be effective until the Respondent is delinquent in two ~ ~ (2) child support payr~ents and has failed to pa~~ a11 arrearages and past public ~ assistance obligations. Service or this order on the employer b~ the office of ~ Child Support Enforcement of the Florida Departr~ent of Healtti and Rehabilitative ~ Services shail be prima facie evidence that the above conditions have occurred. ~ L~'pon receipt of this order, the employer shall govern hir~self accordingly and ~ remir payment forthwith.
