TAKF NOTICE that LIBERTY MUTUAL INSURANCE COMPANY, workers'
compensation carrier for RINRER MATERIALS CORPORATION, employer
of the above noted plaintiff, ROBERT L. JONES, has paid and
will pay compensation benefits and medical benefits under

Florida Workers' eompensation Law as a result of an accident
that occurred on November 13, 1981, which is the subject matter
of the above styled cause. _
NOTICE IS HEREBY given as required by Florida Statutes and
Claim of Lien pursuant to §440.39, F. S., is hereby asserc~u
against any settlement or judgment that may result in this case
for the payments previously made or that may hereafter be made
f by LIBERTY MUTUAL INSURANCE COMPANY because of the workers'
~ compensation claim. .
