4. RESTORATION OF NAME. LINDA G. BERGENROTH be ~
and she is hereby authorized to assume her former name of

5. MEDTCAL EXPENSES: TED L. BERGENROTH is respon-
sible for and shall pay forthwith all bills for medical ser-
vices incurred on behalf of LINDA G. BERGENROTH on or before
the date of tnis Order.
_ 6. RESTRAINING ORDER. Each of the parties is hereby
enjoined and restrained from interfering with, harassing or
- annoying, the other. - - ~
7. FEES AND COSTS. The Court determines the sum
of $350.00 to reasonable for the wife's attorney for his _
~ services herein and TED L. BERGENR~TH is ordered to pay said
