The undersigned, as guardian of the Person and property

1. The .need for the continuation of this guardianship no longer exists by reason of the fact
that Betty B Titus, died Ma~ 14~ 1984 -
2. Petitioner files herewith a final return containing a full and conect account of aq
receipts and disbursements of ali the ward's property of which petitioner has control since the last
return Cled herein, if any, or from the commencement of the administration of this guardianship if
no prior return has been filed, which return includes a statement of the ward's assets.
