AND MEDICAL BENEFITS UNDER FLORIDA WORKERS
COMPENSATION ACT

RON KENDALL MASONRY, INC., and its Workers' Compensation
servicing agent, 5ELF-INSURED SERVICES, INC., by and through
their undersigned attorneys, pursuant to Fla. Stat. Section
440.39(3)(a), hereby give notice that Plaintiff, MICHAEL J.
McGOYE was, at the time and place he was injured as alleged in

= the Complaint heretofore filed in this cause, in the course and

s scope of his employment and the Employer and servicing agent
~ herein above named have paid to Plaintiff various sums of money
~
~ in satisfaction of their obligation to pay Workers' Compensatin
~
~ benefits, including, but not limited to, temporary and permanent ~
~
~
~ compensation, medical benefits and remedial benefits, and said
~ -
~
~ Employer and servicing agent give further notice herein that
~
future compensation and medical benefits will be paid to Claimant.
~
~ WHEREFORE, RON KENDAL MASONRY, INC., and SELF-INSURED
t SERVICES, INC., pray that this notice be filed in the above-styled
~ cause and that it furth~r be recorded by the Clerk of Court and
t that the same, according to statute, constitute a lien upon any
~
~ recovery by the Plaintiff, MICHAEL J. McGOYE, for the full amount
~ of the benefits previously paid to said Plaintiff under the
