NOTICE OF LIEN FOR PAYMENT OF COMPENSATION
AND MEDICAL BENEFITS UNDER FLORIDA WOR:{ERS
COMPENSATION ACT
CENTRAL FLORIDA TRANSFORMER and its Workers' Compensation
servicing agent, AUTO OWNERS INSURANCE COMPANY, by and through
their undersigned attorneys, pursuant to Fla. Stat. Section
440.33(3)(a), hereby give notice that Plaintiff, WILLIAM GRIER
was, at the time and place he was injured as alleged in the
Complaint heretofore filed in this cause, in the course and
scope of his employment and the Employer and servicing agent
herein above named have paid to Plaintiff various sums of money
in satisfacti~n of their obligation to pay Workers' Compensation
benefits, including, but not limited to, temporary and permanent
compensation, medical benefits and remedial benefits, and said
Employer and servicing agent give further notice herein that
future compensation and medical benefits will be paid to Claimant.
WHEREFORE, CENTRAL ~LORIDA TRANSFORMER and AUTO
OWNERS INS. CO., pray that this notice be filed in the above-sty.Zed
cause and that it further be recorded by the Clerk of Court and
that the sa~ne, according to statute, constitute a lien upon any
recovery by the Plaintiff, WILLIANi GRIER, for the full amount
of the benefits previously paid to said Plaintiff under the
