the decedent, a beneficiary and designated Personal Representative under dscedent's
Last Will . Petitioner's name and address are PHILIP K. BROWN , 20139 Scarlet Court,
` Mt. Clemens, Michigan 48043, and the name and address of Petitioner's attorney are
as set forth at the end of this petition .
2. Decedent, LLOYD D. BROWN , whose last known address was ;
