BEFORE ME, the undersigned authority, personally appeared
MICHAEL R. BOLLINGER, well known to me, who first being duly sworn,
did depose and say:
1. That Affiant is employed by First Citizens Federal Savings
& Loan Association as Vice President.
2, That First Citizens Federal Savings & Loan Association holds
a mortgage on behalf of WILLTAM T, BOZARTH and MILDRED E. BOZARTH that
encumbers the following-described property in St. Lucie County, Florida:
Apartment D-9, LA ENTRADA DEL :~tAR, a condominiuin,
according to the Declaration of Candominium dated
Dec~mber 30, 1972, filed rlovember 6, 1973 at 2:49 P.M.
in Of_ficial Records Book 220, Page 1747 of the Public
Records of St. Lucie County, Florida, together with all
of its appurtenances aceording to the Declaration.
3. That WILLIAM T. BOZARTH and MILDRED E. B~JZARTH executed the
above-referenced mortgage on or about October 14, 1974 and said mortgaoe
was recorded in 0. R. Book 232, Page 2672 o.f the Public Records of St.
~ Lucie County, Florida.
! 4. That First Citizens Federal Savings & Loan Association hereby
releases ~JILLIAM T. BOZARTH from any and aIl liability under that certain
; note and mortgage referenced above, dated October 14, 1974 and recorded
