RESIGNATION OF GUARDIAN AND PETITION FOR DISCHARGE,
AND PETITION FOR APPOINTMENT OF SUCCESSOR GUARDIAN
The undersigned Petitioner alleges:
l. Petitioner is the duly qualified, acting guardian of the
property of ELMBER SMITH, incompetent.
2. Petitioner wishes te resign and be relieved of her
duties as such guardian and therefore hereby submits her
resignation.
3. Notice of this resignation has been given to ELMER SMITH
as set forth in the attorney's certificate below. ~
WHEREFORE, petitioner requests that appropriate orders be
entered:
(a) Determining what additional notice, if any, the court
may require.

'4 (b) Finding that the interests of the ward will not be
; placed in jeopardy by petitioner's resigr.ation.
~ (c) Accepting petitioner's resignation.
~ (d) Appainting a successor guardian.

~ (e) Relieving petitioner of her duties upon the appoin*ment
