PETITION FOR APPOINTMENT OF SUCCESSOR GUARDIAN ~
The undersigned petitioner alleges:
1. Petitioner's residence is 133 South Second, Fort Pierce,
FL 33450 and petitioner's post office address is 133 South
Second Street, Fort Pierce, FL 33450.
2. ELMER SMITH, is an incompetent, whose date of birth is
November 5, 1916, who is 69 years of age, and whose social j
security number is 403-12-0389. The residence of the incompetent i
is Port St.Lucie Convalescent Center, Port St.Lucie, FL 33452 and
the post office address of the incompetent is Port St.Lucie
Convalescent Center, Port St. Lucie, FL 33452.
3. The nature of the incompetent's incapacity is repeated
admissions to the hospital because he is unable to care for
himself at home and refuses placement in a protective
environme.t, i.e. nursing home, as adjudged by the Circuit Court
in St. Lucie County, Florida on May 18, 1984.
4. It is necessary that a successor guardian of the person
i and property be appointed for the incompetent.
