PETITION FOR ANCILLARY SUMMARY ADMINISTRATION
Petitioner, C AROLYN LEVINE, alleges:
1. Petitioner has an interest in the above estate as nominated
personal representative in the domicilliary proceedings in~the State
of Massachusetts. Petitioner's name and address are CAROLYN LEVINE,
28 Graham Street, Springfield, Massachusetts 01119 and the name and
address of petitioner's attorney are set forth at the end of this
petition.
2. Decedent, PAULINE MIRKIN, whose last known address was 15
Noel Street, Springfield, Hampden Massachusetts 01108, and whose age
was 96 and whose social security number was 011-50-0595 died on September
4,1983 at Springfield Massachusetts and at the time of her death, the
decedent was domiciled in the State of MassachLSetts.
3. Pursuant to the Last Willand Testament of the deceased, a
P copy of which is attached hereto, the deceased disposed of certain
