summons the Former eusband to appear and give a sworn
stRtement as to his current employm~nt atatus and sources of
income. The Former 8usband may be summoned to appear before
the Clerk by Sheriff's service or by registered mail, return
receipt requested, and the cost thereof shall be reimbursed
by the Former eusband. Failure of the Former Husband to
appear before the Clark after being properly summoned, shall ~
be deemed in contempt of court and shall be punished as such.
IT IS FURTHER ORDERED AND ADJUDGI3D, that the child
support payments ordered herein are subj~ct to further orders
of this Court for income deduction pursuant to Sections
61.1301 and 61.181, Florida Statutes 1984. Opon the payment
of any support ordered herein becoming more than thirty (30)
days, the Clerk of this Court ahall promptly apply for an
Order of Income Deduction, if appropriate, or enforce the
terms of this Order by any civil or criminal remedies
available by law.
4. Medical and Dental Expenses of Children:
The Former ausband, in a~dition to the support
money hereinabove mentioned, during the term he is obligated
to pay said support, shall be responsible for maintaining and
~ keeping in full force, a medical insurance policy, with the ~
` minor children listed as beneficiaries thereon, covering
~
major medical, dental, physciatric, and hospitalization
~ expenses, reasonably incurred for and on behalf of the minor
children, sai8 Former Husband being solely responsible for
~
any c3eductible which may apply prior to payment by the
insurance company. In the event said medical insurance
policy does not cover the medical expenses of the minor
children in their entirety, each of the parties hereto are
ordered to pay one-half of said overage.
g 5. Extent of Modification:

The aforesaid Separation and Property Settlement
Agreement, dated February 16, 1981, shall remain in full
