THIS CAUSE having come on for trial upon the pleadings filed herein and aIl
parties having receive~ proper and timely notice; the Coc;rt having heard testimony and/or
considered the pleadings, papers, affidavits and other papers filed herein, and being ,
otherwise fully and well advised in the premises, it is
ORDERED AND ADJUDGED as foZlows: ~
1. That the minor child(ren) JEFFREY RICHARDO JACKSON, d.o.b. 11/10l78
, is
declared to be the legitimate child(ren) of the Defendant, ~~REW RICHARDO JACKSON
and CARLENE LAVERNE CROWELL , the natural mother. ~
2. That the natural mother, CARLENE LAVERNE CROWELL . sha31 have custody of ,
said childlren) sub~ect to the Def da s right of reasonable visitation.
3. That commencing on ZS~', 19 the Defendant/Fat r shall pay

or cashier's check. All money orders and cashier's checks shall bear the payee's name '
and Social Security number and shall be made payable to the CLERK OF CIRCUIT COURT, and

Said acnount shall be remitted monthly by the Clerk to the bepartment of Health and '
Rehabilitative Services, Child Support Enforcement Unit, 1317 Winewood Boulevard,
Tallahassee, Florida, 32304.
4. That the Clerk of Circuit Court shall and is hereby ordered to continue tQ
transmit support payments received from the Defenflant until further order of this Court
or receipt of a Notice to Discontinue Payments from the Department of Health and
Rehabilitative Services, in which the support payments shall thereafter be directed and
payable to the aforesaid natural mother or person having custody of the child(ren).
5. That the Respondent is additionally ordered to pay total N/A Costs in the
amount of $_7~,56 made payable to: Department of Health and Rehabilitative Services,
1505 Delaware Avenue, Fort Pierce, Florida, 33450, within 60 days from the date of
this Order.
6. That the above-named Defendant having been adjudicated the father of the
above-naned child(ren), the DEPARTMENT OF HEALTH AND REHABILITATIV~ SERVICES, BUREAU OF
VITAL STATISTICS, AMENDMENT UNIT, sha11 and it is hereby ordered to amend the above-named
child's/children's birth certificate(s~ to show the above-named father's name.
7. That pursuant to Section 409.2574, Florida Statutes (1983), that the
employer of the Defendant shall deduct the sums herein provided on a periodic basis from
the income due the Defendant and remit same, after deducting its charge for handling, to
the Central Governmental Depository. Such income deduction shall not be effective until
the Defendant is delinquent in two (2) child support payments and has failed to pay ali
arrearages and past public assistance obligations. Service of this order on the employer
by the Office of Child Support Enforcenent of the Florida Department of :iealth and
Rehabilitative Services sha11 be prima facie evidence that the above cor.ditions have
occurred. Upon receipt of this order the employer shall govern himself accordingly and
remit payment forthwith. This income deduction order shall be in addition to, not in
lieu of, all other remedies provided herein or hereafter.
8. That the child support payments ordered herein are subject to further
orders of this Court for income deduction pursuant to Sectioti b1.181, Florida Statutes
(1984). Upon the payment of any support ordered herein becomming delinquent more than
thirty (30) days, the Clerk of Court sha11 promptly apply for an order of income
