THIS CAUSE having come before the Court on the Complaint to Determine
Paternity of a child born to RANDy A~'~1N LESESNE , natural mother, and to
WILLIE LEE FRr~IvKLIN , putative father; and the DEPARTMENT OF HEALTH AND
REHABILITATIVE SERVICES having requested child support and reimbursement of past AFDC
payments and the Court being fully and well advised in the premises, it is
ORDERED AND ADJUDGED that on the 16th day of April , 19 79 ,
this Court ordered WILLIE LEE FRANKLIN to pay child support for ~
SHERRY LYNN FRANKLIN, d.o.b. 4/6/7$
fn the amount of 25.00 _ per week , It is
further
ORDERED AND ADJUDGED that in addition to the minor child(ren) set forth in the
above-mentioned Order, the minor child(ren) _GWENDELYN LASHA FRANKLTN, d.o.b. 12/24/80;
ANTONIO LESESNE,_d.o.b. 1/17/83
is/are declared to be the legitimate child(ren) of the Defendant WILLIE LEE FRANKLIN
and RANDy ANN LESESNE , the natural mother; it is further
ORDERID AND ADJUDGED th$t the natural mother, RANDY ANN LESESNE shall
have custody of the said child(ren) sub,ject to the Defendant s right of reasonable
visitation. It is further ~
ORDERED AND ADJIIDGED that commencing on the ~ day of ~`Z ,
19 the Defendant father shall pay child support for and on behalf of the
child ren) set forth in this Order and for the child~ ren) set forth in the previous
support order in the amount of $ y~ ~~c~ per V-' ,~oi~c plus statutory fee.
This payment shall include all child support pay~ents the Defendant has been ordered to
pay in this case and prior child support orders are hereby modified as set forth herein.
All payments shall be made in cash, money order or cashiers check. All money orders and
cashiers checks shall bear the payee's name and social security number and shall be made
payable to the CLERK OF CIRCUIT COURT, and sent to:

Said amount sha21 be remitted monthly by the Clerk to the Department of Health and
Rehabilitative Services, Child Support Enforcement Unit, 1317 Winewood Boulevard,
Tallahassee, Florida, 32304. It is furthet~
ORDERED AND ADJiJDGED that the Clerk of Circut Court shall and is hereby
ordered to continue to transmit support payments received from the Defendant until
further order of this Court or receipt of Notice to Discontinue Payments from the
Depart~ent of Health and Rehabilitative Services, in which the support payment sha1Z
thereafter be directed and payable to the aforesaid na[ural mother or person having
custody of the child(ren). It is further
QRDERED AND ADJUDGED that the above-named Defendant having been adjudicated
the father of the above-named child(ren), the DEPARTMENT OF HEALTH AND REHABILITATIVE
SERVICES, BUREAU OF VZTAL STATISTICS, AMENDMENT UNIT, shall and it is hereby ordered to
amend the above-named child's/children's birth certificate(s) to show the above-named
father's name. It is further
OItDERED AND ADJUDGED pursuant to Section 409.2574, Florida Statutes (1983),
that the employer of the Defendant shall deduct the sums herein provi.ded on a peric+dic
