Before me, the undersigned authority, personally appeared
THOMAS A. SMITH who, upon beinq duiy sworn, r~eposes and c~rtifies
as follows:
1. Affiant's name is Thomas A. Smith; he is a practicinq
certified public accountant in Paim Beach County, Florida ar~d is
e~~ the trustee~/ of the Richard A.. Skripak, D.D.S.. P.A.,
Defined Ber~efit Plan and Trust datzd November 1, 1977.
2. The said Richard A. Skripak, D.D.S., P.A., Defined
Benefit Plan and ~'rus~, together with the First Amendment and
Second Amendment thereto, is on file in the offices of affiant
located at 96 N.E. 4th Avenue, Delray Beach, Fl~rida 33444.
3. Said Trust has not been revoked.
4. Pursuant to the terms of said Trust, the trustees have
all of the duties, powers and responsibilities granted to trus-
tees under the Uniform Trust Administration Law (Chapter 737,
Florida Statutes) and the Uniform Principal and Ir_come Law
(Chapter 738, Florida Statutes). ~
5. Said Trust agreement further provides that parties
dealing with the trustee shall be under no obligation to see to
the proper application of any money paid or property delivered to
the trustee or to inquire in~o the trustee's authority as to any
transa~~ion.
