BEF'ORE ME, the undersigned notary public, personally
appeared JAMES P. MAKIELSKI, who, being duly aworn, says that he
is the President and aqent of the lienor herein, whose address
is Blaze Mechanical, Inc., 8503 South Indian River Drive, Fort
Pierce, Florida 33482; and that in accorda~ce with a contract
with Jupiter Development Corporation, Iienor furnished labor,
~ervices, or materials consisting pf all labor and materials
necessary to do the plumbing and fire sprinkler work on the
following described real property in St. Lucie County, Florida:
Legal Description: See Schedule "A"
attached hereto and made a part hereof
owned by Jupiter Beach Corporation of a total value of
$275,250.00, of which there remains unpaid $52,089.00, and
furnished the first of the items on June 3, 1985 and the last of
the items on April 14, 1986; that the lienor served its notice
to owner on June 27, 1985 by certified mail, return receipt
requested; and ~hat the lienor served.copies of the notice on
the contractor on June 27, 1985 by certified mail, return
receipt requested.
