Dear Sir:
The undersigned does hereby apply to the Fort Pierce Utilities ~
Authoritq for en extenaion of vater service and/or sewer service outside 5
the City Limits of the City of Fort Pierce, Florida, to the folloving ;

I understand that in order to obtain extension of such water #
service and/or sewer service,- that I will be required to pap~the cost of
cons~truction of such water 2ine and/or sewer line from point of ~
connection with existing lines of the Uti2ities Authority to my
property. I further agree to deposit with the Utilities Authority,
prior to the time of construction of said line, the total estimated cost
of engineering and construction of said lines. Final payment to be ;
payment in fu1l of actual costs incurr~d. If I choose the opcion of
designing and constructing said lines at my expense, all subject to the
Utilities Authority specifications, no deposit will be necessary. I
further understand I will paq any refund due to others to connect to an
eaisting water or sewer line paid for by others if an existing agree-
ment betveen the Utilities Authority and others so mandate. I further
nndexstand that I will be obligated to_ pay all meter fees and connec- .
tion charges required under the Resolutions of the Utilities Authority
for such service prior to the installation of said lines, and that I
will pay when billed for all water furnished to my propertq aad/or
sewer service according to the rates estab2ished by the Utilities
Authority for the service Z receive. ~ ~
I further understand that if I have not commenced construction
of the wa:er line and/or sewer line requested within one pear from the
date of application, then such epplication shall become null and void
and it wi12 become necessary for me to initiate a new application for
, such water service and/or sewer service.
I understand that the eatension of said uster service and/or
sewer service to my property shall be contingent upon the ability of the
Utilities Authority to supply the demand requested by me vithout in any
way interfering with the water service and/or sewer service naw
perfarmed by the Utilities Authority for the inhebitants of [he City of
Fort Pierce, Florida.
