1. ?he need for the continuation of this guardianship no tonger exists by reason of the fact

2. Petitioner files herewith a fnal return containing a ful! and conect account of all ~
receipts and disbursements of all the ward's property of which petitioner has control since the tast
return filed herein, if any, or from the commencement of the administration of this guazdianship if
no prior return has been filed, which return includes a statement of the ward's assets. .
3. Petitioner represents that the personal representative has
waived notice of hearinu on the petition for discharae of guardian.
(Set forth parties upon whom notice is required or recite waivers. Rule 5.680)
WHEREFORE, petitioner requests that an order be entered approving atl accountings not
previously approved and discharging petitioner as guardian and releasing the surety on petitioner's
bond, if any, from further tiability.
