AFFIDAVIT OF LOST INSTRUMENT

BEFORE ME, the undersigned suthority, personally
appeared FRED KELLER, as Trustee and individually, who upon
his oath deposes and says the following:
1. Tha} a certain Mortgage instrument dated '
September IS, 2961 executed by Bella Vista, Inc., a Florida
corporation in favor of Fred Keller, as Trustee, and
recorded in Official Record Book 36's. Page 654, Public
Records of St. Lucie County. Florida, has been lost,
- misplaced, or destroyed.

2. That the Promissory Note in the principal sum of
$237,000.00 securing the aforesaid Mortgage has been paid
. in full thereby negating any lien of mortgage upan or
pertaininq to the entire parcel of real property as
described in the Satisfaction of Mortgage, duly executed by
Fred Keller, as Trustee.
3. That Affiant, his heirs, successors. assigns, or
transferees in interest make no claim of any kind or nature
against Bella Vista, Inc.. and/or the real property
described in the the Satisfaction of Mortgage and/or
arising under, from. or out of that certain Mortgage
