Pursuant to Florida Statutes and in accordance with the attached

copy of this Court's order as a payroll deduction and you are required

If your payroll is not paid on a weekly basis, then you may adjust
g this amount to correspond to your payroll frequency, but you must be
sure that 26.00 is deducted for each week This shall
` be in addition to any other agency's request for payroll deduction.
The monies shall be sent to the CLERK OF THE CIRCUIT COURT,
= P.O. Box 700, Ft. Pierce, Florida 33454, ATTENTION: SUPPORT SECTION.

~ You must indicate the employee's name, the case number and the employee's

j, social security number on each payment to insure proper credit. You
j~ are allowed to deduct an additional one dollar ($1.00) from the employee's

pay and retain the same as your handling cost for each payment.

This Order shall be binding until further order of this Court

~ You should be advised that this employee can be neither discharged

nor otherwise disciplined because of this income deduction.
