Plaintiff s Complaint filed in this cause, and that CIGNA has paid varidt3s `

sums to or on behalf of the Piaintiff, FREDERICK EARLY, for compensation

and medical benefits, and further gives notice that they have continued ~

to make such payments of compensation and medical benefits, and expect
to make further payments in accordance with the Florida Workers' Compen- r

~ sation Act, Chapter 440 of the Florida Statutes. Payments to date are com-

pensation in the amour~t of $45, 646. 76 and medical benefits in the amount
of $42, ~49. 04. ~
WHEREFORE, CIGNA prays that this Notice be recorded
and in the same constitutes a tien upon any recovery by the Plaintiff for ~
the full amount of the benefits which have ben paid in the past and for the T`
full arnount of the benefits which vticill have been paid in th~ future, pur-
suant to the Flroida Workers' Compensation Act. ~
