number is 266-20-1222 and DAVID SMITH wliich the lien is filed against, date of Birth November
3, 1974. According to the Health Department the DAVID SMITH in question, was only two years
old at time of the filing of the lien.
3. That DAVID ST. ELhiO SMITH is not-one'~nd the same as that DAVID SMI7H in case number
76550MM, whose date of birth is June 30, 1954.
4. That DAVID ST. EtMO SMITH is not one and the same as that RAVID Sh1ITH in case number
76428M, whose date of birth is November 18, 1946.
