THIS CAUSE having come on for trial upon the pleadings filed herein and all ~
parties having received proper and timely notice; the Caurt having heard testimony and/or ;
considered the pleadings, papers, affidavits and other papers filed herein, and being ~
otherwise fully and well advised in the premises, it is, ~
ORDERED AND ADJUDGED as fallows: ~

and WILLIE MAE McPHEE , the natural mother. ~

Sa~si amount shall be remitted monthly by the Clerk to the Department of Health and
Rehabilitative Services, Child Support Enforcement llnit, 1317 Winewood Boulevard,
Tallahassee, Florida, 32304.
3. That the Clerk of Circuit Court shall and is hereby ordered to continue to
transmit support payments received from the Defendant until further order of this Court
~ or receipt of a Notice to Discontinue Payments fro~ the Department of Health and -
, Rehabilitative Services, in which the support payments shall thereafter be directed and ~
payable to the aforesaid natural mother or person having custody of the child(ren). ~
4. That the above-named Defendant having been adjudicated the father of the
above-named child(ren), the DEPARTHENT OF HEALTH ANA RENABILITATIVE SERVICES, BUREAU OF ~
VITAL STATISTICS, AMENDMENT UNIT, ahall and it is hereby ordered to amend the above-named
child's/children's birth certificate(s) to show the above-named father`s name.
S. That it is hereby ordered pursuant to Section 443.051, F. S. (1985), and
; Section 462(e) of the Title IV-A of the Social Security Act that the Department of Labor
` and Employment Security shall deduct and withhold from the Unemployment Compensation
otherwise payable to the Defendant SQX of the Unemployment Compensation or the amount of
child support as ordered above whichever equals the greater amount but does not exceed
the court ordered support amount.
6. This Court reserves jurisdiction for the purpose of determining the amount
due from the Respondent to the Petitioner, if any, as reimbursement for past AFDC
payments received by or on behalf of the child(ren) named herein.
~ 7. Additionally, it is further ordered that Respondent/Payor shall promptly
~ notify the Department of Health and Rehabilitative Services of a12 changes in his or her
mailing address, and a11 changes in the name and address of his or her employer within
~ seven (7) days of such change.

The Court finds tha t t he O b l igor has access at a reasona b le rate to group
health insurance. It is. thereupon ordered and adjudged that said Obligor shall, in
addition to all other terms of this Order, provide health insurance for the child(ren)
