THIS CAUSE having come on for trial upon the pleadings filed herein and all
parties having received proper and timely notice; the Court having heard testimony and/or
considered the pleadings, papers, affidavits and other papers filed herein, and being
otherwise fully and well advised in the premises, it is, i
ORDERED AND ADJUDGED as follows: ~
1. That the minor child(ren), DERRICK RAYMOND HUDSON d.o.b. 1/29/71 :

declared to be the legitimate child(ren) of the Defendant, RUSSELL KENDRICK ,
and REBECCA HUDSON , the natural mother. i

Said amount shall be remitted monthly by the Clerk to the Department of Health and ;
Rehabilitative Services, Child Support Enfarcement Unit, 1317 Winewood Boulevard,
Tallahassee, Florida, 32304. }
3. That the Clerk of Circuit Court shall and is hereby ordered to continue to
transmit support payments received from the Defendant until further order of this Court
or receipt of a Notice to Aiscontinue Payments from the Department of Health and
Rehabilitative Services, in which the support payments shall thereafter be directed and
psyable to the aforesaid natural mother or person having custody of the child(ren). ~
4. That the above-named Defendant having been ad~udicated the father of the
above-named child(ren), the DEPARTMENT OF HEALTH AND RENABTLITATIVE SERVICES, BUREAU OF
VITAL STATZSTICS, AMENDMENT UNIT, shall and it is hereby ordered to amend the above-named
child's/children's birth certificate(s) to show the above-named fath~r's name.
5. That it is hereby ordered pursuant to Section 443.051, F. S. (1985), and
Section 462(e) of the Title IV~A of the Social Security Act that the Department of Labor

and Employment Security shall deduct and withhold from the Unemployment Compensation
• otherwise payable to the Defendant 50X of the Unemployment Compensation or the anount of ~
child support as ordered above whichever equals the greater amount hut does not exceed
, the court ordered support amonnt.
6. This Court reserves ~urisdiction for the purpose of determining the amount
due from the Respondent to the Petitioner, if any, as reimbursement for past AFDC
payments received by or on behalf of the child(ren) named herein.
7. Additionally, it is further ordered that Respondent/Payor shall promptly
notify the Department of Health and Rehabilitative Services of all changes in his or her
mailing address, and all changes in the name and address of his or her employer within
seven (7) days of such change.
