Any e~loyer which ceases to emplo}' or pay the obligor, shall notify the Departme~nt of Health and Rehabilitative Servi~es and shall also pravide Che obligor's lasr imown address and the ha~i~ ~~u address of obligara new employer, i£ imawn. Failure to pravide such inforn~ation may sub3ect the eu~loyer to the civil penalties set forth in paragrapn 9 belaw. 9. It is unlawful for an e~loyer to discharge~ refuse to e~loy~ or • take disciplinary activn against an emplayee because of an Incorne Deduction (?rder. If such actian is taken, the e~loyer may be sub3ect to a civil per~alty not to exceed $250.00 for the first violation and $504.00 for each subseqv~ent violatian. Additianally, the employer may Be co~m~elled by a court of ccxYnet~s?t ~urisdicCion to rehire the emQloyee and pay that employee all back wages and benefits lost, plus reasanable attorney fees and court costs. 10. If the e~aplayer fails to deduct and pay the aIIrnmt ordered by this court for child support payments, it may be liable for the aarn.mt that should have bee~n deducted and paid, plus reasonable attorney fees, court costs and interest. 11. If an e~loyer receives Income De~ction Orders far two or more e~layees requiring that payments be made to the Glerk of the Circuit C,o~tfft of the s~e crnmty, the est~loyer may combine the pa~ts in a single check plus a praper accrnmting of the a~nnts attributable to each employee. In the event more than ane Income Deductian Order is received for an obligor, you st~all c~tact the c~ourt f for further instructions. ~ 12. The e~loyer may collect ~ver and abave the support de~ctian, up to ~ $5.00 for ae~dnistrative costs for the first inc~me deducCion payment for an ; enplayee and $1.00 for each subsequent incane deductian paym~nt.
