THIS CAUSE having come on for tr~al upo~ the pleadings filed herein and all ~
parties having received proper and timely notice; the Court having heard testimony and/or
considered the pleadings, papers, affidavits and other papers filed herein, and being
otherwise fully and well advised in the premises, it is,
ORDERED AND ADJUDGED as follows:
1. That the minor child (ren) , DESMOND DE RArID WALKER d, o. b. 7/ 1/ 86
, is
declared to be the legitimate child(ren) of the Defendant, ROOSEVELT ~JALKFR .
and TAt~ITF TAtIISE MACK , the natural mother.
2. That commencing on ~ ~t-~- 3 , 19 , the DefendantJFather shall
pay chi d support for and on behalf of said child(ren) in the amount of $,3o•c~d per
l~~ , plus statutory fee and a like amount during each succeeding time period until
said child(ren) are no longer dependent under Florids Law . All payments shall be made
in cash. money order or cashier's check. All money orders and cashier's checks shall
bear the payee's name and Social Security number and ahall be made payable to the CLERK
OF CIRCUIT COURT, and sent to:

Said amount shall be remitted monthly by the Clerk to the Department of Health and
Rehabilitative Services, Child Support Enfoxcement Unit, 1317 Winewood Boulevard,
Tallahassee, Florida, 32304.
3. That the Clerk of Circuit Court shall and is hereby ordered to continue to
transmit support payments received froa~ the Defendant until further order of this Court
: or receipt of a Notice to Discontinue Payments from the Department of Health and ~
t Rehabilitative Services, in which the support payments shall thereafter be directed and ~
ii
~ payable to the aforesaid natural mother or person having custody of the child(ren).
4. That the above-named Def.endant having been adjudicated the father of the i
~ above-named child(ren), the DEPARTI~NT OF HEALTN AND REHABILITATIVE SERVICES, BUREAU OF
~ VITAL STATISTICS, AMENDMENT UNIT, shall and it is hereby ordered to amend the above-named
~ child's/children's birth certificate(s) to show the above-named father's n~me.
S. That it is hereby ordered pursuant to Section 443.051, F. S. (1985), and
~ Section 462(e) of the Title IV-A of the Social Securi~y Act that the Department of Labor
s and Employment Security shall deduct and withhold from the Unemployment Compensation ;
~ otherviae payable to th~ Defendant SOX of the Unemployment Compensation or the amount of '
~ child support as ordered above Whichever equals the greater amount but does not exceed !
the court ordered support amount.
~ 6. This Court res@xvea ~urisdiction for the purpose of detexmining the amount
~ due from the Respondent to the Petitioner, if any, as reimbursement for past AFDC
payments received by or on behalf of the child(ren) named herein.
~ 7. Additionallq, it is further ordered that Respondent/Paqor shall prom~ptly
~ notify the Department of Health and Rehabilitat~ve Services of all changes in his or her
` mailing address, and all changes in the name an~~address of his or her employer within
~ seven (7) days of such change.

health insurance. It is, thereupon ordered and adjudged that said Obligor shall, in
