. BEFORS MS, the undersigned authority-personally appeared,
SUSAN M. JOHNSON, who first being duly sworn on oath and states as
followss
1. That Affiants name is SUSAN M. JOHNSON. ~
2. That Affiant ~ioes not have and has never had an ;nterest
in that certain business known as MUNCH MOBILE II, located at 1921
N. Federal Hwy. Ft. Pierce FL 33450.
3. That Affiant does not and never has owed the State of
Florida Department of Revenue any Delinquent Sales & Use Tax.
4. That Affiant has reviewed that certain Warrant for
Collection of Delinquent Sales & Use Tax dat~d M~y 5, 1983, filed
May 6. 1983 in O.R. Book 400, Page 1586 of the Public Records of
St. Lucie county, Florida, as~d Affiant has no knowledge of said
warrant.
5. That the purpose~of this Affidavit is to establish for
the record that Affiant SUSAN M. J08NSON, a single woman, is not
the same person as the Susan Johnson who appears in the
aforementioned Warrant.
FORTHER AFFIANT SAITH NOT.
