THIS CAUSE having come on for trial upon the pleadings filed herein and all
parties having received proper and timely not3ce; the Court having heard testimony and/or
considered the pleadings, papers, affidavits and other papers filed herein, and being
other~•ise fully and well advised in the premises, it is.
ORDERED AND ADJUDGSD as follows:
1. That the minor child(ren), AFNATA MF.RR?TT d o_h__1,~~fRS

declared to be the legitimate child(ren) of the Defendant, .
and STEPHANIE A. 1~IARRITT , the natural mother.
2. That commencing oa APRIL 10 , 19 87 . the Defendant/Fathar shall
pay child support for and on behalf of said child(ren) in the amount of $ t S_nn per
WRRK , plus statutory fee'~and a like amount during each gucceeding ti~e period until
said child(ren) are no longer dependent-under~Florida Law . All payments shall be made
in cash, money order or cashler's check. All ~oney orders and cashier's checks shall
bear the payee's name and Social Security number and shall be nade payable to the CLERR
` OF CIRCUIT COURT, and sent to: .

t Said amount shall be remitted monthly by the Clerk to the Department of Health and
Rehabilitative Services, Child Support Enforcement Unit, 1317 Winevood Boulevard,
~ Tallahassee, Florida, 32304.
3. That the Clerk of Circuit Court shall and is hereby ordered to continue to
~ transmit support pay~ents received from the Defendant until further order.of this Court
or receipt of a Notice to Discontinue Payments' from the Department of Health and
Rehabilitative Services, in which the support payments shall thereafter be directed and
payable to the aforesaid natural mother or person having custody of the child(ren).
4. That the above-named Defendant having been adjudicated the father of the
above-named child(ren), the DEPARTMENT OF HEALTH AND REHABILITATIVE SERVICES, BUREAU OF
VITAL STATISTICS, AMENDHENT UNIT, shall and it is hereby ordered to $mend the above-named
- child's/children's birth certificate(s) to ahow fihe above-named father's name.
5. That it is hereby ordered pursuanC to Section 443.051, F. S. (1985), and
Section 462(e) of the Title IV-A of the Social Security Act that the Departnent of Labor
and Employment Security shall deduct and wiChhold fram the Unemployment Compensation
otherwise payable to the Defendant SOX of the Unemployment Compensation or the amount of
child support aa ordered above Whichever equals the greater amount but does not exceed
the court ordered support amount. !
6. This Court reserves jurisdiction for the purpoae of determining the ainount .
due from the Respondent to the Petitioner, if any, as reiaburs~ment for paBt AFDC
payments received by or on behalf of the child(ren) naued herein.
~ 7. Additionally, it is further ordered that Respondent/Payor shall promptly
notify the Department of Aealth and Rehabilitative Services of all changes ia his or her
~ mailing address. and all changes in the name and addrees of his or her emploqer ~+ithin
~ seven (7) days of euch change. ,

The Court finds that the Obligor has access at a reasonable rate to group
health insurance. It is thereupon ordered and adjudged that said Obligor shall, in
addi~ion to all other terms of this Order, provide health insurance for the child(ren)
~ ~15.00 per week plus statutory fee of $1.00, for a total of $16.00
