granted leave to take the video tape deposition of the DeFendant,
Dr. Hooshang Hooshmand, on June 26, I987, pursuant to the Notice

of Taking Video Tape Deposition which the Plaintiff served on the ~

Defendant or on such other date and time as the Plaintiff may ~
;
designate in a- subsequent notice.
2. Pursuant to the Florida Rules of Civil Procedure and - {
Judicial Administration, it is further ordered that the Plaintiff
i
can take the video tape deposition of Dr. Hooshang Hooshmand,
M.-D., in this case for use at trial in this cause.
3. That the Plaintiff shall serve copies of all future
natices, pleadings, motions and the like upon the Defendant,
VALERIE CULLFNS, by mailing the same to her, postage pre paid, by
United States Mail, at her address of 1008 North 27th Street,
Fort Pierce, Florida, 33450. Proof of service shall be made by
the Plaintiff filing a Certificate of Service with a certificate
of mailing, or by return of service by a process server,
whichever the Plaintiff chooses.
4. WILLIAM LEONARD, certi£ied video tape operator, is =

i designated as the person before whom the deposition shall be
f
~ taken, in addition to a certified court reporter to be designated
~ '
tiy the Plaintiff in the appropriate notice. The original video
.
tape will remain in the custady of BILG LEONARD who will provide ~
copies to each party upon request. Each party will pay for its ~
~ !
~ own copy. €

~ 5. The stenoqraphic transcript will be the official record ;
t ~
of the deposition. ~
6. The camera will maintain a full head shot and will not
zoom in on the deponent unless necessary to focus on an exhibit.
7. The witness will be sworn in on camera.
~ 8. The cast of the video taping will be and the initial ex-
i pense of the Plaintiff.

9. Other procedures may be utilized at the video taping of
i
~ the deposition if counsel for both parties stipulate in writing
~
~ or on the record to such proc:eedings.

~ 10. In order to establish a pretrial schedule for the
