3(C) TO SATISFY THIS ORDER. PGEASE NOTE THE ABOVE DATES AND
APPLY THEM AS SET FORTH ABOVE.
4. Said sum shall be withheld from the sources of
income including, but not limited to, the following:
renumeration for present or past employment, commissions and
bonuses, retirement, benefits, pensions, worker's cQmpensation,
dividends, zoyalties or trust accounts. No monies shall be
withheld from VA benefits or unemplayment compensation.
The total deduction on any one occasion including
administrative charges, may not exceed the maximnm allowable
under the Consumer Credit Pratection Act, 15 U.S.G. 1673(b), as
applicable to the Obligor (50$ of disposible~income where the
Obligor has a second family, 60$ where there is no second
family and an a8ditional 5$ of either limit if the arrearage is
equal to 12 weeks or more in support payments). This case
does, does not qualify for the additional 5$. If the
~ amount to be deducted would otherwise exceed these limits,
' you are to reduce the amount forwarded to the depository
accordingly.
€ 5. The Obligor is notified of his rights, remedies
~ and duties under this Order, as follows:

~ (B) This Order applies to current and
P subsequent employers or periods of employment.

~ (C) This Order will be served upon your
~ employer unless you file a Notice of Contest with the Court
~ within 15 days of the date of this Order. A copy of the Notice
must also be mailed to the Clerk of Court and all parties to
~ this action. The enforcement of the Order may only be
~ contested on the ground of mistake of fact regarding the amount '
~ of support owed, the amount of arrearages, or the identity of
~ the Obligor, and for no other reason.

~ - (D) You must notify the Clerk of the Circuit
Court and alI parties to this action in writing of any change
in your address, employer, or employer's address within seven
days of such change.

Each payment shall include the obli~ or's name, social security
