INCOME DEDUCTION ORDER EFFECTIVE UPON DELINQUENCY
NOTICE TO PAYOR AND STATEMENT OF OBLIGOR'S RIGHTS, REMEDIES
AND DUTIES
: Concurrent with this Court's entry of an Order of Support
and pursuant to Florida Statutes Section 61.1301 (1986 Supp.)

z ORDERED AND ADJUDGED as follows:

- 1. Effective at such time when the Obligor has failed to
" pay child support in an amount equal to one month's obligation,
; and upon receipt of a Notice of Delinquency, any Payor (employer I
~ or income provider) of the Obligor, pursuant to Paragraph 3 herein,
~ shall deduct from all monies due and payable to the Obliyor, the
i sum of $ 75.00 every week and a like sum every
week thereafter, which sum represents the Obligor's
regular current support obligation. Additiona~ly , the Payor shall
deduct the applicable Clerk's Service Charge of $2•~0 for
a total of $77.00
i 2. If the Payor_ is served with a Notice of Delinquency,
Payor shall deduct an additional sum equal to $10.00 per week
until the arreared is paid .in full. At such time as the arrearage
has been eliminated by such payments, then the current payments
shall return to the amolunt set forth in Paragraph 1.
~ 3. Ttie amount required under the terms of this Order and
other orders pertaining to ~hild support and/or alimony payments,
are as follows:

4. Said sum shall be witliheld from the sources of income
including, but not limited to, the following: renumeration
for present or past employment, commissions and bonuses, retirement
benefits, pensions, wbrker's compensation, dividends, royalties, or
