Petitioner, as per~onal representative of the above estate, a~Ieges: i
1. The decedent, MICHAEL RUSIGNUOLO, SR., a resident of St,~LUCie
County, Florida died on July 9, 1986, anc~ letters of administration were
issued to petitioner on September 25, 1986. .
2. Petitioner files herewith either a Final Accounting containinq a
complete report of all cash and property transactions since the last accountin
in this action and since the administration of this estate, or waivers
signed by all in~erested persons, ather than petitioner, waiving the filing
of a final accounting.

3. Petitione~- has fully administered this estate by making payment ~
settlement, or other disposition of all claims that were presented, and
by paying or making provision for the payment of alI taxes and expenses
of administration.
4. The amount of compensation paid or to be paid to the personal
representative, attorneys, accountants, appraisers or other agents
employed by the personal representative is set forth in Exhibit A attached
hereto.
5. Petitioner has filed all required estate tax returns with the
Internal Revenue Service and with the department of RevenUe of the State
of Florida, and has obtained and filed with this court evidence of the
satisfaction of this estate's obligations for both Federal and Florida
~
estate ~axes, if any. ~
6. Petitioner has made or proposes to make distribution of the assets
of this estate as set forth in the schedule attached hereto as Exhibit B.
7. The only persons, ather than petitooner, having an interest in this
estate and their respective addresses are:
