The undersigned petitioner alleges: ~ ~
1. Petitioner's residence is 3417 Abbott Street, Ft.
Pierce, Florida.
2. Daniel Eugene Yates is a minor whose date of birth was
June 18, 1974 who is 13 years of aqe, and whose social security .
nwnber is 408-27-0505 , The residence of the incompetent is 3417
Abbott S~reet, Ft. Pierce, Florida.
3. It is necessary that a quardian of the person and
property be appointed for the incompetent.
4. The approximate value and description of the incompetent's
property are: the net settlement proceeds from a jury verdict of
$150,000.00 rendered in the case of 7~ateB v. Nicholson, Case No.
83--737-CA-10, St. Lucie County, Florida.
5. The names and addresses of the persons most closely

~ related to the incompetent are: Samuel Yates, father and Gloria
' Yates, mother, both residing at 3417 Abbott Street, Ft. Pierce,
