The undersigned Ptitioner alleges: v a,
. ,
1. That the Petitioner's residene is 367 Yardley
Terrace, Port St. Lucie, Florida, although he is presently
residing at New Medico Rehabilitation Center, Wauchla, Florida.
2. That DAVID TITUS, the Petitioner herein, is 28 years
of age, having been born on November 14, 1958, and his Social
Security Number is 209-52-37$2.
3. Petitioner, although mentally competent, is
incapable of care, custody and mangement of his estate due to
incapacity resulting from injuries sustained in an automobile
~ accident, and he is unable to car,~ for his normal affairs or for
~ ,
~ his own normal personal needs and voluntarily petitions this
Court for appointment of a Guardian of his personal property
pursuant ~o the.applicable provisions of the Florida Statutes.
4. That this Petition is accompanied by an affidavit
from a 13.censed physician who h~s treated the Petitioner and has
found tbat the Petftioner is con~petent to understand the nature o£
the guardianship and his delegation of authority. ~

5. That it is necessary that the Guardian of the I
