assets of your Petitioner 3s insurance settlement proceeds in the
approximate sum of $20,000.00.

Lucie because Petitioner is a permanent resfdent of this County.
8. That your Petitioner's mother, SHEILA JOHNSON, is the
person most closely related to Petitioner and Petitioner desires
that she be appointed as h~s Guardian in this matte~.
9. That the~Petitioner's mother, SHEILA JOHNSON, is sui
juris and otherwise qualified under the laws ~f the State of
Florida to act as Guardian for the Petitioner's estate and she is
requested to be entitled to preference by virtue of this request
of the Petitioner.
WHEREFORE, the Petitioner requests that his mother,
SHLILA J08NSON, be appointed as Guardian of his estate without the
necessity of filing any bond and without the necessity of any
formal heari~q.
