office address is Post Office Box 82, Fort Pierce, Florida 34950.
Petitioner, PAULINE ACKLEY'S residence is
408 Oleander Avenue, Fort Pierce, Florida 34952, and Petitioner's
post office address is the same.
2. OLIVE B. GILBERT is an incompetent, whose date of
birth was February 15, 1899, who is 88 years of age, and whose social
secu=ity number is 265-58-9201. The residence of the incompetent is
3614 Okeechobee Road, Fort Pierce, Florida 34950, and the post office
address of the incompetent is the same.
3. The nature of the competent`s incapacity is senile
dementia, as adjudged by.the Circuit Court, Nineteenth Judicial
Circuit, Fort Pierce, St. Lucie County, Florida, on October 02, 1987.
4. It is necessary that a guardian of the person and
property be appointed for the incompetent.
5. The approximate value and description of the incompetent's
