security number is 174-14-8410 died on the 14th day ot July, 1986
at Smethport, Pennsylvania, and at the time of. death decedent was
~ donniciled in Smethport, McKean County, Pennsylvania. ;
~ 3. So far as i~ known, the names of the beneficiaries

~ of this estate and the decedent's surviving spouse, if any, their
~ addresses and relationship to decedent, and the ages of any who
are minors, are:
