6. That pursuan~ to Section 443.051. Florida Statutes ~
(1985) and Section 462te) of the Title IV-A of the Social-
Security Act that the Department of Labor and Employment Security
shall deduct and withhold from the Unemployment Compensation
other~aise payable to the Defendant 50X of the • Unetaployment
Compensation or the amount of child support as ordered above
whichever equals the greater amount but does not exceed the court
ordered support amount.
7 Additionally, it is further ordered that the
Respondent/Payor shall promp tly notify the Clerk of Court of gll
changes in his or her mailing and residence, and all changes in
the name and address of his or her employer within seven (7) days ~
of such change. ~ '
8. That this Court reserves jurisdicCion for the
purpose of determining the amopn ~c~ue from the Itespondent to the
Petitioner, if any~ as reimbu~ement of past AFDC payments
received by or on beha2f of the before-nawed child(ren).
9. That in the event the Defendant/Obligor becomes
unemployed~ he/she shall seek employment and he/she sh811
. cooperate with the Department of Labor and Emploqtaent Services of
the State of Florida and make reports to the Depattment of Health
and RehabilitaCive Services of the State of Florida. Child
Support Enforcement Unit, of his/her efforta to maintain
employnent~ on a weekly basis.

~ reasonable rate to grou health insurance. It is thereupon
ordered and adjudged that said Obligor shall. in addition to all
other terms of this Or r, provide health insuzance for the
~ child(ren) set forth herein for so Iong as the child(ren) are
dependant under Florida law. The Obligor shall file proof of
said health insurance coverage in this file and send a copy to
' aIl parties within 15 days of the date of this order.
