amend the above-x~amed child's/children's birth certificatets) ta
show the above-named father s name.
6. That pursuant to Sectinn 443.051. Florida Statutes
(1985? and Section 452(e) of the Title IV-A of the Social
S~curity Act that the Department of Labor and Employsaent Security
shall deduct and withhold from the Unemployment Cotapensation
otherwise payable to the De£endant SOZ of the Unemployment
Compensation or the amount of child support as ordered above
whichever equals the greater amount but does not exceed the court
ordered supporC amount.
T~.~_ .Additionally~ it is furthPr ordered that the
Respondent~Payor shall promptly notify the Clerk of Court of all
changes in his or her mailing and residence, and all changes in
the name and address of his or her employer within seven (7) days
of such change.
8. Thet this Court reserves jurisdiction for the
purpose of determining the amount due from the RespondenC to the
Petitioner~ if any. as reimbursement of past AFDC paymenta
received by or on behalf of the before•named childtren).
9. Tl~at in the event the DefendanCJObligor beeomes
unemployed~ he/she shall seek employment and he/8he at~all
cooperate with the DepaYtment of I.aboz and Employment Services of
the State of Florida and make reports to the Dep~rtment of Health
and Rehabilitative Services of the State of Florida, Child
Support Enforcement Unit~ of his/her efforts tp maintain
emplo}craent, on a weekly basis.

- The Court finds that the Obligor has access at a
reasonable xate to group ealth insurance. It ig thereupon
ordered and adjudged that s id Obligor shall. in addition to all
other terms of this Order., provide health insuZanca for the
child(ren) set forth herein for so long as the childtren) are
dependant under Florida law. The Obligor shall file proof of
said health insurance 'coverage in this file and send a copy to
all parties vithin 15 days of the date of this order.
