'IYiat the above-named Defendant having been adjudicated the fa[her of the above-named child(ren), the Department of Health and Rehabilitative Services, Buresu of Vital Statistics, Amendme~t Unit, shall and it is hereby ordered to amend the ebove-aemed child'slchildren's birth certificate(s) to sho~ the above-mm~ed father's name. , 6. ~at pursuant to Sectio~ 443.051, Florida Statutes (1985) anci Section 462(e) of the TitYe N-A of the Social Security Act that the Department of Labor ar~d bnployinent Security shall deduct ard t~ithhold froan th+e Urx~nployment Compensatic~n othen~ise payable to the Defendant S0~ of the Unemployment Compensation,or the mnau~t of ch31d support as ordered above ~ahichever equals the greater amount but dces not exceed the court ordered support amount. 7. Additic~ssally, it is further ordered that the RespondentlPayor shall promptly notify the Clerk of Court of all changes in his or her mailing and residence, and all chariges in the name and address of his or her ~nployer Within seven (7) days of such change. That this Court reserves jurisdi.ction for the purpage of detexminining the amount due from the kespondent to the Petitioner, if arry, as reimbursement of past AFDC payments received by or on beha3f of the before-~narned child(ren). 9. ~at in the event the Defe~dat~t/Obligor becomes unemploy+ed, heishe shall seek employaient and he/she shall cooperate With the Department of Labor and ~nployaaent Services of the State of Florida and make reports to the Depart- ment of Health and Rehabilitative Services of the Stete of Florida, Child Support Enforcement Unit, of his/her effor,ts to maintain employment, on a weekly basis. The Obligor shall file proof of said health insurance coverage in this file and send a copy to all parties within 15 days of the date of this order.
