• DEPARTMENT OF HEALTH AND REHABILITATIVE SERVIGES, BUREAU OF VITAL
STATISTICS, AMENDMENT UNIT. shall and it ts hereby ordered to
amend the above-named child's/children's birth certificate(s) to
show the above-named father's name.
6. That purauant to Section 443.051, Florida Statutes
(1985) and Section b62(e} of the Title IV-A of the Social
Security Act that the Departtaent o£ Labor and Employment Security
shall deduct and withhold from the Unemployment Compenestion
otherwise payable to the Defendant SOx of the Unemployment
Compensation or the amaunt of child supporC as ordered above
whichever equals the greater amount but does not exceed the court
ordered support amount.
7, Additionally, it is further ordered that the
Respondent/Payor shall promptly notify rhe Clerk of Court of all
changes in his or her mailing and residence, and all changes in
the name and address of his or her empZoyer within seven (7) days
of such change.
, 8, That this Court reserves ~urisdiction for the
purpose of determining the amount due from the Respondent to the
Petitioner, if any, as reimbursement of past AFDC paytaents
received by or on behalf of Che before-named childtren).
9. That in the event the Defendant/Obligor becomes
unemployed, he/she shall seek employment and he/she shall
cooperate with the Department of Labor and Eaaployment Services of
the State of Florida and make reports to the Department of Health
and Rehabilitative Services of the State of Florida, Chi2d
Support Enforcement Unit, of his/her efforts to maintain
employ~ent~ on a weekly basis.

- The Court finds thaC the Obligor has access at a
reasonable rate to group heaZth insurance. It is thereupon
ordered and adjudged that said Obligor shall. in addition to all
other terms of this 4rder, provide health insurance for the
child(ren) set forth herein for so long as the child(ren) are
dependant under Florida law. The Obligor shall file proof of
said health insurance coverage in this file and send a copy to
all parties within 15 days of the date of this order.
