STATISTICS, AMENDMENT UNIT~ shall and it is hereby ordered to
amend the above-named child's/children's birth certificate(s) to
show the above-named father's name.
b. That pursuant to Section 443.051~ Florida Statutes
(1985) and Section G62(e3 of the Title IV-A of the Social
Security Act that the Department of Labor and Employment Security
shall deduct and withhold from the Unemployment Com~ensation
other~ise p8yable to the Defendant 50~ of the Unem~loyment
Compensation or the amount of child support as ordered aUove
whichever equals the greater amount but does not exceed the court
ord~t~_{~ u?pport ~mount.
Additionally ~ it is further orciereJ that the
Respondent/Payor shall promptly notify the Clerk of Court of all
changes in. his or her mailing and residence. and all changes i.n
the name and address of,~h'i~s or her employer within ~even (7) days
of such change. -
8. That t~is Court reserves jurisdiction for the
purpose of determining the amount due from the Respondent to the
Petitioner, if any~':as reimbursement of past ArDC payments
received by or on behalf of the be£ore-named child(ren).
9. That in`: the event the Defendant/Oblig,or becor.ies
unemployed, heJshe stia~l s~ek employment and helshe shall
. cooperate with the Departm~nt of Labor and Fmployment Services of
the State"of Florida and make reports to the Uepartmcnt of Iic~.~lth .
and Rehabilitative Services of the State of ^loricia, Child
Support Enforcement Unit, of hisjher efforts to r~.~intain
employment, on a weekly basi~.

ordered and adjudge.d'that said Obligor shall, in aclditio~l to ~11

dependant under FZarida law. The Obligor shall file proof of
said health insurance coverage in this file aiid send a copy to
