THIS CAUSB came on to be heard upon the Sti~lation ~

of the Former Husband and the Former Wife and the Cour~ being

fully advised in the premises; it is hereupon; ~
ORDBRSD AND ~DJUDGBD that paragraphs 2 and 3 of the
Final Judgment of Dissolution of Marriage entered on February 17, j

1983 shall be modified to read as follows: i
2. CUSTODY: The parties shall have shared parental
responsibility of ANTHONY H. HURLEY as that term is presently
defined by Florida law and each shall fully participate with the
other and the child in all important matters pertaining to the
child's health, welfare, education, and upbringing, with a view
toward arriving at a harmonious policy calculated to promote the
child's best interest and not with a view toward the personal
r desires of the parties. It is understood that each shall fully

E participate in all major matters and decisions involving the
~
p child, such as, but not necessarily limited to; health care,
~
~ including the selection of doctors and dentists, the education of
~
~ the child, the religious upbring and reliqious institutional
~
membership, travel attendance at camp, and the like.
The primary physical residence of the minor child shall
~ be with the Husband with the Wife havin liberal and reasonable

visitation with the minor child, pursuant to the standard Child
Visitation Guidelines attached hereto. All expenses of
s
transportation of the minor child to and from the Wife's
residence shall be paid by the Wife.
~ Each party shall at all times keep the other party
f
~ advised as to where the minor child shall be, whether during
~
~ visitation, ~acation, holidays, or otherwise, and provide the
~
~
~
