THIS CAtISE having come on for trial upon the pleadings filed herein and all parties ~
h.t~~ing received proper and timely notice; the Court having heard testimony and/or considered
cti~~ pleadings, papers, affidavits and other papers filed hsrein, and being otherwise :

ORDERED AND ADJUDGED as folloWS:
1. That the Respondent pay to the Petitioner for the benefit of

3. That the Clerk of Circuit Court shall and is hereby ordered to continue to
~i-ansmit support payments received from the Defendant until further Order of this Court or
i-eceipt of a Notice to Discontinue Pay~ents from the Department of Health and Rehabilitative
~ervices, in which the support payment shall thereafter be directed and payable to the
~foresaid natural mother or person having custody of the child(ren).
4. That the above-named Defendant having been adjudicated the father of the
jbove-named child(ren) the DEPARIMEN~' OF HEALTH AND REHA$ILITATIVE SERVICES, BUREAU OF
t~ITAL STATISTICS, AMENDMENT UNIT, shall and it is hereby ordered to amend the above named
=;:il~'s/children's birth certificate(s) to show the above named father's name.
5. That it is hereby ordered pursuant to Section 443.051, F.S. (1985), and
~ection 462(e) of the Title IV-A of the Social Security Act that the Department of Labor
3nd Fmployment Security shall deduct and Withhold from the Unemployment Compensation
;~therwise payable to the Respondent SOx of the Unemployment Compensation or the amount
of child support as ordered above whichever equals the greater amount but does not
exceed the court ordered support amount. This Unemployment Compensation ded~ction order
;hall be in addition to, not in Iieu of, all other remedies provided herein or hereafter.
