ORDERED AND ADJUDGBn that pursuant to Section 443.051,
Florida Statutes (198~) and Section 462(e) cf Title IV-A of the
Social Security ~1ct that the Department of Labor and ~:mployment
Security shall deduct and withhold from the unemployment compen-
sat ion, otherwise payable to the Respondent SUS of the unemployment
compensation or amount ot child support as ordered above, which-
evpr equals the greater amount bui. does not exceed the court
ordered supp~rt amount. It is further
ORDER~ AND ADJUDG~ that this Court reserves jurisdiction tor
the Nurp~se of determining ti~e amount due from the Responnent to
the Yetitioner, if any~ as reimbursement fur past AFDC payments
received by or on behalf of the child(reii) named herein. It is

OBDERED AND ADJUDGBD that the Respondent/Obligor shal:i
promptly notify the Departaent of Healt6 and Aehabilitati~e Ser~ices of t6e of
all changes in hia or her ~niling addrese, and all changea ia ns~e and addresa

said healch insurance in this file and send a copy to all parties
