The above-named Defendant, having been declared
indigent, or partially indigent, end said Defendant having
received, at his request, the assistance of the Public Defender's
office or the services of a private attorney appointed purauant
to the Florida Statutes or the Florida Rules of Criminal
Procedure, and said Defendant having been determined to be guilty ~
of a criminal act, and said Defendant having been advised of `
