i 2. Decedent, JOSEPH S. GREBIK, whose last known address
~ was 616 SE Degan Drive, Port St. Lucie, Florida, and, if known,
whose age was 74 years old and whose social security number is
~ 381-07-3868, died on December 8, 1989 at HCA Medical Center, Port
St. Lucie, Florida, and at the time of death decedent was
domiciled in St. Lucie County, Florida.
3. So far as is known, the names of the beneficiaries of
~ this estate and of the decedent's surviving spouse, their
~ addresses and relationship to decedent, and the ages of any who
